Services Offered

Core Services:

Linkage to the community through resources and referral: The YFRC serves as a community services hub by providing a one-stop access point and clearing house of community services and health education. The YFRC operates on the belief that every community has assets and resources, and when they are strategically used they can lead to effective and appropriate services to individuals, families, and the community. The challenge is to identify and match resources and the people who need them. During calendar year 2008, the YFRC made over 6800 referrals to community services and served over 10,000 unduplicated clients.

Case management – working directly with individuals, children, and families in providing assistance, supportive programming, and linkage to other service providers and resources.  Families receive assistance in locating and obtaining supports through a variety of community resources and programs in order to increase family functioning, stability and success. Case managers work closely with all other personnel, including counselors, to provide comprehensive, coordinated services. Case managers conduct school and home visits, as well as meet with clients on site. Staffs are trained in case management practices and are bilingual in English and Spanish. Supportive Services:

Parent education, training and support:  The YFRC serves as the community access point  to 1)  refer or direct parents to services, 2) provide critical parenting and child development information and services, and 3) provide parent education programs.

Health insurance enrollment – assist families in health insurance enrollment.

Health education – provide access to health information, education, and services.

Financial literacy and family economics – provides classes in basic financial planning, budgeting, credit issues, and banking.

Life skills –provides a range of enrichment activities  and skills development trainings f              or parents, youth, and children.

Support groups – in such areas as parenting, family issues, special needs children, child care and nutrition.

Food Stamp enrollment – an enrollment site for families to apply for food stamps.

Youth development and engagement activities –provides a range of programs and activities to support youth leadership, skills development, and participation in supportive activities.

Homeless prevention services:   pending

Basic Needs Referrals — access to referrals and resources to assist families in meeting their basic needs, includes PG&E payments, shelter, STEAC and food bank referrals.
